- One pair of professionally fitted contact lenses, a comprehensive eye exam and any required training is available for the low cost of $155.00.
** Hard, gas permeable contact lenses are available at an additional low fee.
- Items may be purchased separately: Eye exam-$50.00, Frame-$40.00, Single Vision glasses-$49.00 and Bifocal glasses- $59.00.

- A select network of Eye Care Providers is conveniently located in your neighborhood.
- We provide primary eye care vision with no hidden costs. No monthly premiums and no deductibles.
- An administrative fee of $ 12.50 (individual)/ $ 18.75 (family of 1 or 2 parents and up to 3 children)/ $ 35.00 (families with more than 5 members) is charged only when services are rendered. We strongly recommend that you make your payment through the internet BEFORE
visiting your Eye Care Provider. An additional fee will be
charged if payment is made in the provider's office.
- Most routine optometry procedures and optical goods are available to members.
- Pediatric, Low Vision and Specialty lenses are available.*
